I need to know the required documents for extending my D-2 visa.

I've been feeling unwell and have been so busy that I just realized my D-2 visa's entry period expires next week... I haven't even made an entry/exit appointment yet. Could someone with experience please tell me what documents I need and what to do if I can't get an appointment through Hikorea? I'm going to Central University.
